I'm not sure how this works for us. Our tax is added in with our mortgage and the bank calculated it with the discount, when we set up the mortgage. So I need to e-mail them and see if we have to complete the forms or whether they have done them on our behalf. Thanks for the reminder.

I just got my property taxes this past week. I contribute with my mortgage each month throughout the year too. If you don't get the form in the mail, contact City Hall and they'll either send it out to you, or at least give you the code that Rich mentioned to submit online. Even though the mortgage company takes off the tax portion, you still have to submit the application for the grant each year - the mortgage company will contact City Hall and make the necessary payment.
On the sheet I have, it says if your name doesn't appear on the notice (past owner's I gather)... you are required to provide a Registered Form of Freehold Title Transfer to City Hall.

Thanks for the info. We have the forms, we just haven't done anything with them yet. I think I'll use Rich's link and do it over the internet, it is so much easier, plus if it is like the census you get a confirmation number to prove you have done it.

So long as tax is paid up for the current year (i.e. no debt o/s) there is no problem with the rebate.
